Document Required for Private Medical College at the time of Registration

  • NEET PG Registration with Photo or NEET PG Admit Card
  • Higher Secondary (12th) Mark-Sheet (10+2) or Equivalent Mark-Sheet for Date of Birth Proof
  • Mark of MBBS (Third Prof. MBBS Part – I & Part – II). In case of candidate has passed his/her MBBS from Out of India – FMGE passing Result (MCI Screening Test)
  • Internship Compulsory Certificate issued by competent authority
  • Registration with Madhya Pradesh Medical / Dental Council or receipt of application applied for registration
  • Any ID Proof of the Candidate – Driving Licenses/Aadhar Card/Votar ID/Pan Card / Passport etc.
  • Certificate issued by Competent Authority for Economy Weaker Section (EWS) Candidate for the financial year 2021-22.
  • M.P. Domicile Certificate issued by competent authority
  • Caste Certificate issued by competent authority with reservation in the State of Madhya Pradesh
  • Income Certificate issued by Tehasildar / Nayab Tehasildar for the Current Financial Year (for Assessment of OBC Creamy layer / Non-Creamy Layer) If applicable
  • Certificate issued under RPWD Act 2016 for Physical Handicapped Category or issued by Any State or Central Authorized Handicapped Centre
  • Notarized Proforma 3, 4 & 5 for Admission under NRI Quota ONLY
  • Notarized Seat Leaving Bond Proforma – 2
  • Notarized Rural Service Bond Proforma – 2
